If you were ill within six (6) weeks prior to your procedure, please call to discuss whether you need to reschedule your appointment. Symptoms include, but are not limited to fever, chills, coughing, shortness of breath, sore throat, running nose, nasal congestion, nausea, vomiting, stomach or bowel issues, muscle pain, headache, and new loss of taste or smell.

Please do not eat or drink (including water) for eight (8) hours prior to the appointment. This includes gum, candy and breath mints.

If your appointment is in the afternoon, you may have a light breakfast 8 hours prior to the appointment.

To keep you safe and to follow physical distancing, our lobby is now your car. When you arrive for your appointment and are parked outside our office, please call 224-770-3001.

If you are a minor (less than 18 years old), you must be accompanied by your parent or legal guardian. If you are an adult, a responsible escort/driver, 18 years or older is required to accompany you to the appointment and check in. Out of concern for your health, other patients being seen in the office and the oral surgery team, we kindly ask that only one individual escort you and both you and your escort wear a mask while in our office. After checking in, your escort will wait in the lobby or their car for the duration of the appointment. On the morning of the appointment, we ask that you and your escort both check your temperature and ensure you do not have any of the cold symptoms listed above. If you record a temperature of 100°F or above, feel ill or have cold symptoms, please call our office to reschedule.

If you smoke, we ask that you quit smoking several weeks prior to surgery to decrease the risk of complications.

Please wear comfortable, loose fitting clothing, short sleeve shirt and flat shoes. Please bring a sweater as our office temperature is very cool. Please do not wear slippers or flip-flops.

Do not wear lipstick, excessive makeup, or nail polish on the day of surgery. Acrylic nails, contact lenses, jewelry, and dentures must be removed prior to surgery.

If you take routine oral medications, please check with Dr. Shinozaki prior to your surgery for instructions. If you have an inhaler or Epi pen, please bring it, and make sure it is not expired. Please pick up any medications prescribed by Dr. Shinozaki prior to your appointment. If a premedication was prescribed, please follow the provided instructions.

Intravenous medications administered during the procedure have negative effects while being pregnant. Please confirm there is no chance of being pregnant prior to your appointment. 

If you need to cancel or reschedule your appointment, please call us as soon as possible so that we may care for another individual. To avoid an appointment cancellation charge, please contact us at least 48 business hours prior to your appointment.
