First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. A second appointment will be scheduled for surgery.
Prior to your consultation appointment:
- At your earliest convenience, please complete the patient registration form by clicking the link on the right.
- Please email your referral form and any x-rays to [email protected]
On the day of your consultation appointment:
- Please bring a list of your current medications.
- Please bring a form of identification along with your medical and dental insurance cards.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.
A preoperative consultation and physical examination is mandatory for patients undergoing treatment.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., blood thinners, osteoporosis medication, heart medication, anti-resorptive medication, aspirin, bisphosphonate, etc.).
X-Rays
If your dentist has taken x-rays, please have them forwarded to our office. If additional x-rays are necessary, they can be taken at our office.
